HOW TO NOMINATE A CHILD FOR A DREAM AWARD

STEP #1

Before midnight March 31, a parent may nominate a terminally ill or permanently disabled child for a Dream Award. All nominees must be within either the Houston or Dallas Fort Worth Metropolitan Areas.

STEP #2

Our selection committee will inform parents of awardees of our choices by midnight April 3, 2019 - three days prior to the I45 Push Up Challenge.

STEP #3

All Dream Awards will be issued from Texas nonprofit Boss Life Foundation based on funds raised during the I45 Push Up Challenge.

Construction Costs

Funding for a child's special needs such as Access Construction

Travel Costs

Funding for a trip which might be a child's dream

Medical Costs

Funding for a needed medical procedure or for a medically prescribed device which is not covered by insurance

qualifications and definitions

Child's Age

Any child between the ages of one and seventeen (1-17) years of age meets the age qualifications for the I45 Challenge. Proof of age may be required.

Child's Terminal Illness

Terminal illness a medically diagnosed incurable disease that cannot be adequately treated and in some instances can reasonably be expected to result in the child's death.

Child's Permanent Disability

A medically diagnosed physical or mental condition that limits the child's movements, senses, or activities.

Child's Parent

The mother or father of the child or the legally designated representative(s) of the child.

Selection Committee

Committee made up of qualified screeners from Houston and Dallas Fort Worth assigned the task of reviewing nominee applications. Committee may request additional information to further guide its decisions.

Boss Life Foundation

A Texas-based Nonprofit with current tax exemption status under Internal Revenue Code Section 501(c)(3). Tax exemption documentation provided to all private and corporate donors.
